How to Submit a Notary Name Change in Florida

To make a notary name change in Florida, just follow these requirements:

COMPLETE THE AMENDED COMMISSION REQUEST FORM:

If the Florida Notary Association is your current bonding agency, complete our Amended Commission Request form. If you are not bonded through us, contact your bonding agency for assistance.

PREPARE THE REQUIRED DOCUMENTS:

Along with your Amended Commission Request form, include your current commission certificate. If the certificate is lost, provide a signed letter stating so.

PAYMENT:

Remit a payment of $52 with your request. This fee covers the cost of processing your name change and issuing a new notary stamp and certificate. You can make the payment via check, money order, purchase order, or credit card.

MAIL ALL DOCUMENTS:

Send everything to the Florida Notary Association at P.O. Box 533964, Orlando, Florida 32853.

RECEIVE NEW STAMP & CERTIFICATE:

Expect your new stamp and amended commission certificate within approximately ten business days. Continue performing notarial duties under your former name until you receive your updated stamp.

How to Submit a Notary Change of Address

To make a notary address change in Florida, follow these steps:

  • Fill Out the Change of Address Request: Our Notary Address Change form can be filled out online or printed. You will need your name, date of birth, commission number, expiration date, and email address to complete the form.

 

  • Submit the Form: You can submit your form online, or it can be mailed, emailed, or faxed to Florida Notary Association. Here are our contact details:

  • Yes, you must surrender your notary commission if you move out of state. Unfortunately, you cannot transfer your Florida notary commission to another state.

  • No, Florida notaries are not authorized to perform notarial acts outside of Florida.

  • You can renew your Florida notary commission online. Make sure to complete the renewal process before your current commission expires.
